Rehabilitation case management is the process of assessing the injuries and the psychosocial impact upon the individual and their family following a traumatic event. Following the Initial Needs Assessment (INA), a programme is produced to put in place provisions required to achieve an optimum level of recovery while maintaining as much of the individual’s independence and dignity as possible. The effectiveness of this programme is monitored throughout the rehabilitation period to ensure the client is receiving the best care possible.
This service is provided by qualified and experienced health care professionals who work closely with the NHS multi-disciplinary teams.
Ongoing Case ManagementThe LCM Rehab philosophy is to focus not only on the injury but on the entire person and their family. We recognise and understand the impact of serious injury to an individual can have on their family and wider social network. We have the skills within our team to address the physical, psychological and social components of a client’s needs and assess the overall impact of the trauma.
We see ourselves as different from many other case management providers as we work very closely together, drawing expertise from individual members of our team and focussing the knowledge and skills from all the team members into the care of each individual. This also ensures that in the absence of the primary case manager, another team member can step into the role ensuring a seamless provision of care.
Our focus is to establish a strong therapeutic relationship with our clients, their family and carers, gaining their confidence and trust from the beginning of the rehabilitation process. This has proven to help clients engage with treatment and shorten their recovery time.
We also have a reputation of ‘getting the job done’ without any fuss and unnecessary administration. When appropriate, from the time of referral, we can implement an INA assessment, produce a report, gain approval, and have treatment in place within 1-2 weeks.
We work in close partnership with NHS multi-disciplinary teams and private health care professionals, combining these services to provide comprehensive care and treatment.
Throughout the rehabilitation process monitoring takes place to ensure that the treatment standards are high and are working effectively. Regular update reports are provided to inform all parties of the details of the rehabilitation programme and progress being made.

Since starting her own case management company she has created a very cohesive and cheerful team who are known for their no-nonsense policy of ’getting the job done’, and fulfilling the objective of helping their clients to gain the optimum level of improvement efficiently and effectively.

Tim has a wide and varied experience in managing a broad spectrum of personal injury cases.
He is qualified in both general and mental health nursing with a variety of additional training and qualifications and has particular expertise in dealing with the psychological components of personal injury. He is a qualified teacher and trainer and provides in-house training for his colleagues.
Tim has a somewhat unique background and has skills and knowledge of building and industrial settings and can confidently project manage alterations and adaptations to clients’ homes.
Tim likes a challenge and sets his standards high and is always determined to help clients reach their optimum level of recovery.

Pauline is a Registered General Nurse with over 30 years clinical experience, she has held key clinical case management roles in the UK and NZ, she is recognized as having extensive knowledge and experience of complex personal injury assessment and case management.
As a clinician and senior health professional, Pauline is passionate about maximizing every client’s potential and facilitating the best possible outcome for them regardless of disability. She believes, in building on what is good, involving the client and all relevant parties right at the start of the rehabilitation journey.
Pauline is proficient in the provision of complex medical case management in a wide range of injuries. She is skilled in planning and coordinating personalized client centred cost-effective treatment, therapy and vocational reintegration packages.
Pauline has respect for ethnic, cultural, religious differences, including disability and personal beliefs and can be flexible and resilient when dealing with unexpected or challenging situations. She prides herself on having a can-do attitude.

Andy is an experienced physiotherapist working within his own private practice. His specialty is the treatment of spinal cord and head trauma, complex orthopaedic injuries, and amputations. We have had the benefit of Andy’s skills since the start of LCM Rehab and these skills have been invaluable, particularly throughout the Covid 19 situation. He has developed programmes designed to be implemented remotely, which have proved to be effective in assisting clients to make progress. He has also produced video demonstrations for clients to follow at their convenience.
Andy also provides some case management, attending appointments and his knowledge and skills are invaluable when peer reviewing cases.
